A completed (with token) online criminal record check provided by CRC.com that covers the preceding 5 years from the date of application. The criminal record check can be no more than 60 days old. The applicant must also provide no less than five character references when completing the criminal record check.
A completed fingerprint card that is mailed to the Board’s office
A digital headshot photograph
A signed and notarized PPSB Release of Records form
A signed NCSBI Authority for Release of Information (fingerprints) form
Proof of high school graduation, general education certificate, or proof of graduation from an accredited college or university.
A completed PPSB Application for Company Business License (not required for sole proprietorships)
Military veterans or active servicemembers only: a DD214, an NGB-22, or if you are currently serving in the military, a letter of good standing from a commanding officer
Trainee license applicants only: a completed Written Plan of Supervision
Applicants for polygraph, polygraph trainee, electronic countermeasures, psychological stress examiner, close personal protection, or digital forensics examiner licenses may be required to provide proof of having completed a Board-approved school to qualify for the license.
